In only a small fraction of cases were psychotropic drug levels publicly reported in toxicology findings—highlighting a critical data gap that allows potential drug contributions to remain invisible. Tennessee has taken a concrete step to close that gap. Legislation effective July 1, 2025, mandates testing for psychotropic drugs, including at therapeutic levels, in autopsies of individuals suspected of mass shootings resulting in four or more deaths. A 2026 expansion extends provisions to incidents involving four or more injuries or attempted mass killings and allows testing of living suspects with consent. Results are studied by the University of Tennessee Health Science Center and reported to the state legislature. CCHR urges other states and federal authorities to adopt similar requirements so that patterns of drug-related violence can be identified and addressed. These developments occur against a backdrop of growing debate within and beyond the mental health professions. Clinicians, researchers, and patient advocates increasingly question polypharmacy—the simultaneous prescription of multiple mind-altering drugs—and the frequent absence of thorough physical examinations and laboratory testing before and during treatment. Withdrawal reactions from antidepressants, antipsychotics, and benzodiazepines can emerge within hours or days and may include agitation, impulsivity, and suicidal or aggressive ideation. Roger McFillin, Ph.D., a clinical psychologist, recently commented on the practice of polypharmacy as the standard of care in psychiatry, referring to a case where a young mother progressively worsened with multiple prescriptions of psychotropic drugs, with tragic results. He stated: “That is not a scandal within psychiatry. That is psychiatry.” Critics argue that adverse effects are too often misattributed to the patient’s underlying condition rather than the drugs themselves, leading to dose escalations or additional medications that compound the problem. Historical precedents reinforce the need for accountability.
