At the same time, the research highlights persistent unmet need among patients with refractory or difficult-to-treat RA despite the breadth of therapies already available. Rosnilimab Stands Out as a Potential Advance in the RA Pipeline Rosnilimab differentiates itself not only through its novel mechanism, but also through rheumatologists’ assessment of its potential clinical value. Nearly one-third of physicians rate the agent as a substantial advance over current RA treatment options, the highest proportion among the pipeline therapies evaluated. Difficult-to-Treat RA Reinforces the Need for New Treatment Approaches The need for continued innovation is particularly evident among patients with difficult-to-treat (D2T) or refractory RA, where physicians may cycle through multiple mechanisms without achieving adequate disease control. Rheumatologists estimate that 16% of their RA patients are refractory to treatment, a meaningful population given the breadth of advanced therapies already available. Spherix’s qualitative research suggests rheumatologists distinguish refractory RA, generally characterized by failure of multiple biologic or targeted therapy classes, from the broader D2T population. Difficult-to-treat patients may include those with refractory disease, but also patients whose management is complicated by comorbidities, treatment intolerances, psychosocial or access barriers, and overlapping pain syndromes such as fibromyalgia. For these patients, physicians describe treatment journeys that can span TNF inhibitors, JAK inhibitors, IL-6 inhibitors, T-cell modulation, and B-cell-directed therapy, underscoring the limitations of simply cycling through existing options. This challenge may also help explain physician interest in emerging mechanisms. By comparison, just one-fifth anticipate a substantial impact on Rinvoq, while biologic competitors are expected to see relatively little effect. The findings point to an important distinction in the evolving RA market: cost-driven disruption is intensifying within established classes, but physicians continue to reserve their strongest enthusiasm for pipeline therapies that offer a clearly differentiated clinical or treatment proposition.
